Golfers elbow is also known as medial epicondylitis and is the less common sister condition of tennis elbow, both conditions sharing the tendon degenerative nature without inflammation. They are referred to as tendinopathies due to the pathological changes which occur inside the tendon without an inflammatory process. The third to fifth decades of life are the commonest periods for pain onset and 60% of golfers elbow occurs in the dominant hand. An acute onset of pain is reported in a third of patients, with the other two-thirds developing over a period of time.

Typical symptoms are an ache or pain over the front of the inner elbow which is worsened by repeated wrist bending and improved by rest. Hand, forearm, elbow and shoulder pain may be reported, with grip weakness in the hand. If the ulnar nerve is involved this can also contribute to the weakness and give pins and needles in the forearm. The physio palpates the muscles and their tendinous insertions, the elbow joints and the groove behind the elbow where the ulnar nerve lies. To exclude other significant diagnoses the osteopath will perform a neurological examination.

The main treatment of golfers elbow is conservative, including anti-inflammatories, wrist and forearm splinting, corticosteroid injection and osteopathy. There has been much debate in recent years over the negative impacts that high heels can have on your body. Doctors shake their heads in dismay when women come to them with back problems, osteopaths tut, and the local newspaper’s health section goes into a frenzy about the evils of stilettos. But are they really that bad? All right, after you wear them, your feet can be a little painful and your calves might twinge a bit for a couple of days, but once worn on a regular basis, your body does get accustomed to high heels – and your calves and feet will be tougher for it.

It is high time someone put these rumours to rest. Some people have done so already: earlier this year, an article was published by the BBC which argued that high heels do have benefits on the body. Here we shall challenge three of the most common myths regarding stilettos, and proceed to look at the ways in which your body can benefit from them.

Myth #1: High heels give you back problems. Not necessarily, say senior osteopaths. Only people with predispositions for back problems have a higher probability of getting one – but that doesn’t mean everyone is doomed to injure their back as a result of wearing stilettos. While it has been confirmed that walking on 8-inch heels for extensive periods of time can result in too much pressure being put on the front of the ankles (which leads to postural problems), this can be easily avoided if you take care. Instead of walking on stilettos all day, for instance, travel to work in your trainers and only slip on your heels at the office.

Myth #2: Stilettos result in stiffness in the neck and lower back. Pure rumour. Osteopaths note that stiffness has more to do with how you wear the heels – and less to do with the fact that you’re wearing them. If you already have problems with your posture even before put on heels (i.e. titling your back), then your postural predisposition will be aggravated by wearing heels, as they add pressure to the lower back. However, if you make sure you assume a straight posture before slipping on those stilettos, then the problem can be avoided altogether. In fact, it has been argued that heels can actually correct a woman’s posture by forcing her to balance her weight, and therefore stiffness is not a direct result of wearing high heels.

Myth #3: Heels are bad for your muscles in general. Again, a common misconception. Your muscles can become sore by wearing heels excessively, but this can all be avoided as suggested for Myth #1. In fact, a recent study reveals that women who wear stilettos have tighter pelvic muscles and stronger legs. As long as you vary the type of shoe you are wearing every day, you face no danger of overexerting your muscles.

Another interesting advantage of stilettos -as the BBC article revealed – is that they can improve your sex life. The article quotes a study conducted by Italian Urologist Dr. Cerruto in which she explains that the heel forces the foot in an angle so that it exercises and strengthens the pelvic floor functions. A woman’s pelvic muscles are very important to sexual performance, so the notion that intercourse may be improved simply by wearing heels is a welcoming one indeed.

Regardless of what myths or truths you chose to believe about stilettos, the bottom line is that there is always a solution that is right for you. If you do experience long-term problems, do not hesitate to contact your osteopath, but remember that these issues are not necessarily linked with the heels you wear.

Back Pain, Spine Strength, and Good Posture

The spine is probably the single most amazing thing about your body. It supports us as we walk upright and helps us to lift objects. Keeping the spine in good shape is vital to a happy, healthy life. As a sufferer of back pain myself, I have learned a few things that may help you to relieve the pain you have or, hopefully, to avoid it altogether.

When you have back pain, even the most commonplace tasks can become difficult and painful. Something as simple as tying your shoes can become a challenge. However, you don’t have to let back pain hold you back. In order to keep back pain under control, the most important thing is to be able to prevent it ‘ and this all starts with an understanding of how our spines work.

Your spine consists of a series of bones; the vertebra which are arranged in a vertical row which includes three natural curves: the cervical curve (in the neck), the thoracic curve (in the middle of the back) and the lumbar curve (in the lower back).

Between each vertebra are fluid filled discs to cushion the vertebra so they don’t rub together; very painful! Attached to the vertebra are a myriad of muscles.

So, as you can see, the back is a concert of muscle, bone and cushions all acting together. When you are at rest, a good posture is important to keep the spine in its natural position. A good posture is achieved by having the shoulders aligned with the hips. When sitting, the feet are flat to the floor, the knees at right angles. When standing, the feet should be no more than shoulder width.

By maintaining proper posture, you’ll be able to maintain the three natural curves in your spine, which is very important to preventing back pain and keeping your spine and back muscles in good shape. However, what should you do if you already suffer from back pain?

One of the best ways to relieve back pain is through performing simple exercises. These can both alleviate pain that you’re currently experiencing as well as helping to prevent further back pain. Before you exercise or engage in any physical activity, do some bending and stretching to loosen up your back muscles and reduce the chance of straining yourself.

If you are already in pain, there are exercises you can do, like that above, but they need to be done carefully and not overdone. Leg lifts are great for this and so are side bends and forward/backward bends. Remember, though, that if it hurts, STOP IMMEDIATELY!

By warming up before exercise and maintaining good posture, you’ll usually be able to prevent back pain and even relive back pain that you’re already suffering. There are a lot of people who successfully manage to rid themselves of their back pain by simply training themselves to exhibit good posture.

Be good to your back and spine and it will be good to you for a long time to come.

One last tip to help avoid serious back injury and it is the lifting posture. Always lift with your knees, not your back. Bend the knees, grasp the object and just stand up. Works every time!

The Development Of Chronic Back Pain ” Part Three

Corticosteroid injections by epidural or root blocks are commonly used to treat leg and back pain and may indicate that in some cases there may be an inflammatory contribution to the pain. In disc material removed from humans during operation has been found heightened levels of inflammatory chemicals which could be affected by steroid use. These inflammatory chemicals may increase degeneration of discs and sensitise the fibres of the outer disc walls. Some chemicals may also sensitise the discs by elevating reactivity of sensory nerves, inhibit the creation of new proteoglycans which hold water in the discs and may contribute to disc degeneration.

The lumbar spinal motion segments have nerve structures supplying them which can be come hypersensitised to incoming stimuli, creating pain from what would have formerly been normal sensations. Sciatic leg pain is known as nerve root or radicular pain and can be caused by irritation, inflammation or compression of the spinal nerves as they exit the spinal cord. Why nerve root pain occurs has not been established but candidates are an impoverishment of blood supply, inflammatory reactions, axon dysfunction from compression and biochemical agents. The compression that occurs may have greater effects on spinal nerve roots than other nerves and result in more significant internal nerve swelling.

The nerve may suffer from excessive inflow of fluid secondary to compression, which results in internal nerve swelling from the increased pressure. This can slow the flow through capillaries and allow fibrosis to develop in the nerve. Nutrition to the spinal nerve roots comes via the cerebrospinal fluid to the tune of about 60% and nerve fibrosis interferes with this, making the nerve more responsive and perhaps more likely to suffer compression symptoms due to the increased reactivity of the nerves. Blocking or reducing the blood supply can cause pain in these nerves, unlike the more usual loss of motor and sensory functions in compression of peripheral nerves.

Development of nerve roots signs and symptoms is more likely if the nerve suffers compression which comes on quickly rather than slowly which may indicate why many older people, who have compromised nerve exits, do not complain of symptoms. In experimental research work significant inflammation has been noted when disc material is placed in the epidural space of dogs and nerves may suffer damage in contact with disc material. Injection of TNF, tumour necrosis factor, into a nerve mimics the changes which occur in nerve root syndromes. Another contributory cause which has been proposed is an inflammatory reaction of an auto-immune cause but this idea has yet to be substantiated.

The facet joints are synovial joints, like our finger joints, and are formed from articular processes on an upper and a lower vertebra. Facet joints react to injury and damage in the same way as our other joints react, with pain, stiffness, loss of movement, muscle spasm and eventually degeneration. As the discs degenerate the facet joints follow by becoming osteoarthritic. The fibrous capsules which surround the facet joints are rich in nerve endings and injections into the joints have been shown to refer pain into the arms, upper back and legs. Facet joints are capable of generating pain and have been shown experimentally to be involved in significant percentages of patients with neck, thoracic and lumbar pain.

The sacro-iliac joint between the pelvis and the sacrum is a large synovial joint known to refer painful and other symptoms to the groin, the leg, low back and buttock. In patients with chronic low back pain, experimental testing has shown involvement of the sacroiliac joint in their pain problems in from two to thirty percent of cases. Pain receptors exist in muscular tissue also and respond to stretching and pressure as well as other inputs. There is no denying that chronic back pain can be extremely difficult to live with. Whether we are just sitting still or moving about the house, our backs are the key element in performing any task that we want throughout the day. Even the simplest of things become hard to deal with when suffering from back pain.

Quite often, the cause of back pain is so simple that it’s something taken for granted. Poor posture is at fault for most chronic aches and pains in the back. We don’t realize this, however, because we typically associate pain with something that just happened, rather than with behaviors that feel completely natural.

The back is critical to overall health. It supports a large amount of weight and keeps us up-right. Furthermore, numerous nerve endings connect through the spine and associated muscles, muscles which connect throughout the entire structure of the back.

If postural issues are not addressed, serious problems can manifest. If the muscles of the back are under constant strain, you’ll begin to hunch, thus increasing tension on your spine. Tight muscles slowly shift vertebrae out of alignment, throwing your back out of the healthy structure it needs to be must maintain. Stiffness and pain result, and the vicious cycle continues.

The key to avoiding back pain is simple. First, you must perform specialized exercises to strengthen muscles that have weakened due to disuse. This enables your body to assume correct and healthy posture. Next, muscles must be stretched to improve flexibility. This helps ensure that the correct posture, which will feel very unnatural at first, feels much less so. Without stretching, your body will have the strength it needs but will persist in old habits. Also, because these muscles are fairly small, they usually tone up rapidly and, once your body assumes correct posture naturally, the muscles often stay strong without the need for continual specialized exercise.

There is no reason to continue living with back pain everyday. Since the problem can usually be corrected with ease, you need to know that you will not be forced to suffer with your back pain much longer. By taking the first few steps to a healthy back, you will be at your goal of being pain free in no time at all.
